  • Corazon Counseling and Wellness is located at 110 N Main St. Poynette, WI 53955.

  • Online therapy is the option of seeing a therapist without any face-to-face contact, either via video or phone call. I use secure, HIPAA-compliant services to provide therapy. If you have a video visit, you will receive a link via email, which you will click on at the time of your session, either on your smart phone or computer. If you are having a phone call appointment, I will call you at the time of the appointment.

  • Yes, I offer in-person sessions at the Poynette office located at 110 N Main St. Poynette, WI 53955

  • Corazon Counseling and Wellness has a billing support team that will submit insurance claims on your behalf. Clients (or Guarantor) are responsible for confirming benefit coverage prior to first appointment. Corazon Counseling and Wellness requires all clients to keep a credit card and sign a consent form for automatic billing. The card is stored in a secure Electronic Health Record System. Your card will be charged any co-pay, deductible, or missed appointment fee within 24 hours following your appointment.

  • If you do not show up for your scheduled therapy appointment, and you have not notified me at least 24 hours in advance, you will be required to pay the full billed amount of the session, since I am not able to bill insurance for a missed appointment.

  • After you are scheduled, you will need to register in your client portal. The link to this portal will be included in an email titled “Client Portal.” In the event that you did not receive this email (after checking spam and junk), please contact my office and ask that it be re-sent. These forms take approximately 20 minutes to complete. If unable to complete these forms electronically at home, please arrive at your scheduled appointment 20 minutes early to complete them. Failing to do this may result in your appointment being rescheduled.

    These intake forms serve as a contract between you and Corazon Counseling and Wellness, and need to be completed at least 24 hours before your appointment. Please allow yourself adequate time to thoroughly read and complete these forms.

  • Everyone’s circumstances are unique to them and the length of time counseling can take to allow you to accomplish your goals depends on your desire for personal development, your commitment, and the factors that are driving you to seek counseling in the first place.

  • As of January 1, 2022, state-licensed or certified health care providers must provide a Good Faith Estimate of healthcare charges to every new and continuing client who is either uninsured or is not planning to submit a claim to their insurance for the healthcare services they seek.

    There are specific rules for what information has to be in that estimate and when it has to be provided.
