THIS NOTICE DESCRIBES HOW HEALTH INFORMATION MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
I. Our Pledge
We are committed to protecting your Protected Health Information (PHI) and have a legal duty to provide this notice of our legal duties and privacy practices with respect to your PHI. We will follow the terms of the notice currently in effect.
II. Use and Disclosure

III. Authorizations Required
Psychotherapy notes require your written authorization for disclosure, except for specific legal or safety reasons. We confirm that your PHI will not be sold or used for marketing purposes without your explicit authorization.
IV. Disclosures Not Requiring Authorization
We may disclose your PHI without your authorization for public health activities (such as reporting abuse or neglect), health oversight activities, law enforcement purposes, and to provide appointment reminders.
V. Opportunity to Object
You have the right to object to the sharing of your PHI with family members or friends who are involved in your care. If you are not present or unable to agree or object, we may exercise our professional judgment to determine whether the disclosure is in your best interest.
